Fitness for work certificates

You do not require a doctors fitness for work certificate for any illness lasting seven days or less. Your employer may, however, require you to complete a self - certification form (SC1) which is available from your employer or at any of the surgeries. For any illness lasting longer than seven days you will need to see the doctor for him / her to issue a fitness for work certificate (Med 3). Please request any subsequent renewals of the certificate in writing; you will then be informed if the doctor needs to see you again.
